• Either out-of-work needing health-related support to return to work, OR employed and either absent through sickness or struggling in the workplace due to a health condition
• Have a home address OR GP/local Jobcentre Plus address within Bristol, North Somerset, or South Gloucestershire
• Over 16 years old
• Have the right to work in the UK
